The goal of Diabetes Self-Management is to provide the tools needed to take control of your diabetes and live a healthy life. The program requires a physician's referral and is paid by most insurance companies - please contact us for a referral form. Diabetes Self-Management consists of:

• A 1-hour visit with a nurse to learn the basics of diabetes management including how to monitor your blood sugar.
• A 1-hour visit with a dietitian to develop a meal plan that will work for you.
• A series of group classes on all aspects of managing diabetes including:
• Medications
• Preventing complications
• Physical activity
• Monitoring
• Supermarket (grocery shopping) tips
• Stress management
• Follow up visits with the diabetes educator offered
